Stop Smoking Service Client Record Form

This template client record form contains all of the items required for routine data collection for submission to the Health and Social Care Information Centre (HSCIC).

We have also included items which will be of use for practitioners when delivering a behavioural support programme. We are happy for local stop smoking services to adapt this form to meet their needs and those of their clients.
